If you have signed up for meals (breakfast, lunch, or dinner) at the Olympic Center cafeteria and have food allergies, please submit the following confirmation form.
Please down load, fill it out and email it to the Executive Committee.
We will not be able to accommodate requests without documentation.
※If you are not allergic (there are no symptoms) and no action is required, you do not need to submit the sheet.
